Disc Golf Release of Liability, Waiver, and Indemnity Agreement
(“the Release”)
PLEASE READ CARFULLY. BY SIGNING THIS FORM YOU AGREE TO WAIVE OR GIVE UP CERTAIN LEGAL RIGHTS INCLUDING THE RIGHT TO SUE OR CLAIM COMPENSATION FOLLOWING AN ACCIDENT.
I, (the “Participant”) wish to access and use the Disc Golf Course and its surrounding facilities which include, certain washrooms and designated parking areas (collectively the “AHE Facilities”) located at the Alberta Health Services (“AHS”) facility known as the Alberta Hospital Edmonton (“AHE”). By accessing and using the Disc Golf Course and AHE Facilities located on the grounds of the Alberta Hospital Edmonton (”AHE”) for the purposes of playing Disc Golf, and any related events and activities, I recognize and understand the following:
In consideration of AHS allowing me to access and use the Disc Golf Course and AHE Facilities, and for good and valuable consideration, the receipt and sufficiency of which is acknowledged, I hereby agree to the terms and conditions as follows:
- I must complete this Release in full and provide it to AHS before I access and use the Disc Golf Course.
- I have read and understand the contents of the Information Sheet, Schedule A, provided to me regarding the use of the Disc Golf Course on the AHE grounds. I agree to only use the AHE Disc Golf Course during scheduled times, as outlined in Schedule A.
- I am aware that my use of the Disc Golf Course and AHE facilities involves certain risks, including but not limited to: serious injury and illness (eg: communicable diseases such as COVID-19), the potential for permanent disability and death, the potential changing weather conditions; variation or changes in the Disc Golf Course surface including slippery surfaces, holes, etc.; and impact or collision with other participants or discs;
- I recognize and fully understand that the above list is not a complete or exhaustive list of all possible risks; the list only provides examples of types of risks that I am assuming. I knowingly and freely assume all such risks, both known and unknown and assume full responsibility for my participation in Disc Golf, and use of the Disc Golf Course and AHE Facilities;
- I willingly agree to comply with AHE’s stated terms and conditions, attached as Schedule A, for my use.
- TO WAIVE AND RELEASE ANY AND ALL CLAIMS that I have or may have in the future against AHS, its directors, officers, employees, representatives, agents, contractors, successors and assigns, and those for whom AHS is legally responsible (hereinafter referred to as the “Releasees”) from any and all liability for any loss, damage, expense or injury that I may suffer due to any cause whatsoever, including, but not limited to negligence or breach of any statutory or other duty of care on the part of the Releasees, arising directly or indirectly as a result of my participation in Disc Golf, or access to and use of the Disc Golf Course and AHE Facilities.
- TO INDEMNIFY AND HOLD HARMLESS the Releasees from and against all claims, demands, actions, orders, charges, liabilities, expenses, costs, damages or losses which may be brought or made against, or which may be suffered or incurred by the Releasees arising as a result of or in connection with any loss or damage to tangible property, or bodily injury to or death of any person due to my participation in Disc Golf, or access to and use of the Disc Golf Course and AHE Facilities.
- This Release is binding upon myself, my heirs, executors, administrators, personal representatives and assigns.
Definitions
Waive: To abandon, renounce, or surrender (a claim, privilege, right, etc.); to give up (a right or claim) voluntarily.
Negligence: The failure to exercise the standard of care that a reasonably prudent person would have exercised in a similar situation; any conduct that falls below the legal standard established to protect others against unreasonable risk of harm, except for conduct that is intentionally, wantonly, or willfully disregardful of others’ rights. The term denotes culpable carelessness.
Indemnify: To promise to reimburse (another) for such a loss.
Binding: (Of an agreement) that binds (a binding contract). Bind: to impose one or more legal duties on (a person or institution.)
Heirs: Loosely defined – a person who inherits real or personal property, whether by will or by way of laws that apply when a deceased has no valid will.
Executors: A person named by a testator (individual who has a will) to carry out the provisions in the testator’s will.
Administrators: A person appointed by the court to manage the assets and liabilities of a person who has died without a valid will.

SCHEDULE “A”
Alberta Hospital Edmonton Disc Golf Information Sheet
The Niska Paskwahk (Goose Meadow) course will be open 7 days a week between 0900 – 2100h unless otherwise communicated to the EDGA by the AHE Disc Golf Contact person. (ex. special events on site) – to be communicated at least 7 days in advance where able. EDGA members will need to carry their EDGA bag tag to identify they have completed the AHE Waiver. If Bag Tags are lost, follow the established EDGA process for replacement.

Road 1: Overflow gravel parking lot (Northeast)
Road 3: Overflow gravel parking lot (Southwest)
Bathroom Locations
1. 3 Building – Helen Hunley Pavilion: Front (west) entrance at Security Station # 1
2. Sports Field Bathrooms
3. 9 Building Main Entrance (bus stop)
Alberta Hospital Edmonton Site & Disc Golf Course Rules
- Who can use the AHE Site Disc Golf Course
- Members in good standing with EDGS with a signed AHS Disc Golf Waiver
- EDGS members to carry their current bag tag with them
- Minors to be accompanied by an adult
- AHE Patients & their Families
- AHE Staff, Physicians and Volunteers
- Members in good standing with EDGS with a signed AHS Disc Golf Waiver
- All Players must uphold AHS Code of Conduct & AHS Smoke Free Environments while on site
- AHS sites are also free of alcohol and other substances.
- EDGS Member must utilize assigned parking in gravel lots
- EDGS Members must remain in designated areas (ie. Designated parking lots, bathrooms and disk golf course identified on AHS Facility and Disc Golf Course Map)
- AHE is a health care facility. No taking pictures that involve other individuals (patients or staff)
- EDGS Members must uphold the confidentiality of patients and staff on site. Failure to maintain confidentiality will result in immediate termination of the AHS/EDGS License Agreement
- AHE Disc Golf Waiver to be completed annually
- EDGS members shall not bring guests who are not members of EDGS to play on the AHE Disc Golf course.
- Be mindful of safety at any time. If there is an identified safety concern please contact Protective Services for urgent concerns and Recreation Facilities for non-urgent matters.
- Must wear appropriate footwear and clothing mindful of being at a health care facility (ie. No profanity etc.)
- Play in a safe and courteous manner
- Remain quiet and avoid unnecessary movements while others are throwing
- Stand behind the player who is throwing until throw is complete
- Remove disc from disc pole basket after completing hole
- Help new players learn the rules
- Allow faster groups to play through when possible
- Pick up trash and put in proper receptacles
- Do not alter the course (trees, bushes, etc) in any way
