Agreement of Confidentiality

I, undertake, and agree that I shall not disclose to anyone or at any time any confidential client information that I gained from time spent at Alberta Hospital Edmonton.

All information which comes to me from Alberta Health Services files, or from personal interaction with clients, shall be kept confidential at all times and not disclosed by me to any person, with exception of Alberta Health Services personnel.

I understand that failure to adhere to the above may lead to termination of my involvement and leave me subject to legal procedures.
